All patients must have physician orders [recommendations] to attend physical therapy services at Cooper & Bush Physical Therapy.
Many physician offices make the first contact with our clinic, and our staff will then place a telephone call to the patient to schedule the initial evaluation.
When speaking with the new patient, the staff of Cooper & Bush Physical Therapy will obtain demographic and insurance information.
Prior to the first visit, all insurance plans will be verified. Our business office staff will meet with the patient, and the insurance benefits will be reviewed with the patient prior to being seen by the Licensed Physical Therapist.
Some carriers require 'Prior Authorization' before attending physical therapy services. Our staff will work with your insurance company and physical to obtain the authorization prior to the physical therapy services.
Also, the patient will be asked to sign consent forms and designated release of information forms on the first date of service.
